We are currently accepting registration for the COVID-19 vaccine for all qualified persons 5 years of age or older.

How It Works

Pre-registration REQUIRED for PEDS Age 5-11 vaccine. Click here to register for PEDS Age 5-11. Otherwise, for age 12+ appointments are not required, however we encourage pre-registration
Please note, time slots are approximate as our drive-thru site accommodates drive-up patients as well
Once you pre-register, you will need to sign up for Intend with name, email address, and mobile phone number.
Complete the short profile BEFORE you come. This includes your age, sex, address, and a few additional questions, including choice of vaccination and which dose (1st or 2nd)

Be Prepared

Please have your ID and insurance card ready.
Please let us know if you are uninsured. We will be required to attest to you not having insurance in your profile
Please complete REQUIRED CDC questionnaire PRIOR to your visit.

Drive-thru Vaccination

Join the line of cars; please be mindful of the traffic flow, traffic cones and directions provided by staff; please follow signs for COVID VACCINE
Remain in your car and lower window when instructed by staff
Staff will verify ID, insurance and help complete your profile
Chosen vaccine (subject to availability) will be administered by trained staff
You will be asked to wait in a designated parking space for a minimum of 15 minutes; observation may be longer depending on your personal history
Once observation period is complete, without any concerning adverse reaction, you will receive your vaccination card and are free to leave the vaccination site

Walk-up Vaccination

Available for PEDESTRIANS ONLY; please do not park your car and attempt to walk up (you will be asked to return to your vehicle and join the line)
Please note that after administration of vaccine, you will be asked to wait in a designated sitting area for a minimum of 15 minutes; observation may be longer depending on your personal history
